The loss of one or more teeth can make each of us uncomfortable, regardless of age.
Currently, the replacement of missing teeth is no longer a problem. Dental implants are the best solution to replace the natural tooth quickly, without being left with the empty space, during the period of its integration. This is thanks to the immediate loading of the system, which is now widespread throughout the world.
We therefore know two distinct techniques of implant surgery:
- immediate load implantology (in one phase) which is divided into 2 other areas:
– immediate definitive loading implantology;
– temporary immediate loading implantology. - deferred load implantology (in two phases).
But how to make the right choice? Which of these two techniques brings the most benefits? Di seguito proviamo a spiegare in che consiste ogni tecnica, qual è la differenza e quali sono i vantaggi e gli svantaggi di ognuno di esse.
What does immediate loading implantology consist of?
L’implantologia a carico immediato è una tecnica che prevede l’inserimento degli impianti in titanio e l’ancoraggio delle corone o protesi fisse in una sola fase, quindi Le permette di avere denti fissi fin da subito.
There is often confusion, believing that the protection put on after the insertion of the implants is definitive one ( immediate definitive load implantology), this is rarely worn.
In reality, in most cases it is the temporary prosthesis (implantology with immediate provisional load), and in 3-6 months the crowns or the definitive prostheses are anchored.
With immediate load implantology, you can replace a single tooth or an entire arch, provided that the primary stability of the implant (the insertion torque) has the appropriate value, recommended by each implant manufacturer.
Immediate definitive loading and immediate provisional loading. What sets them apart?
In the process of carrying out the immediate loading, according to the technique (temporary or definitive loading), 2 absolutely different types of implants are used (you can see them in the image below):
- monoblock dental implants;
- conventional dental implants (traditional).
For the immediate definitive loading, monobloc dental implants are used, formed from a single piece and already provided for the abutment. Such implants are often placed in larger quantities (around 10-12 implants on a single jaw) if all the teeth on the arch are missing (total edentulism). These implants are loaded with bridge or definitive prosthesis – meaning, the treatment is finished in one phase only (very rarely practiced technique). Quando ci sono ancora dei denti in bocca (edentulia parziale) questi impianti possono essere uniti con i denti naturali da un ponte unico (vedi l’immagine che segue).
Traditional dental implants have 2 components (the implant and the abutment). Also, they allow the patient to have fixed but temporary teeth (usually in resin).
In che consiste l’implantologia a carico differito?
L’impianto a carico differito è una tecnica che si esegue in due fasi. In the first phase fits the titanium implant on which is placed a cap. Then the gum is closed with sutures and in the second phase, the abutment on which the crown is anchored is already mounted. The inter-phase period, also called the osseointegration period, is 3-4 months for the mandible and 5-6 months for the upper jaw.
Even in the case of deferred loading implantology, the patient can have a provisional but this will be a mobile provisional or a fixed bridge that rests on natural teeth and not on the implants.
Is it better to have deferred loading or immediate loading implants?
As mentioned above, immediate loaded implantology with final teeth is very rarely used for good reasons, so below we will refer to the difference between temporary immediate loading implantology and delayed loading implantology.
The advantages and disadvantages of immediate loading implantology or deferred loading implantology must be analyzed individually according to the clinical case, for the success of the intervention.

Reflections and conclusions on immediate and deferred loading implantology
If the patient does not have the necessary conditions for the realization of immediate loading technique, then you can use deferred load implantology.
By choosing the immediate loading implants, the patient gains on the time of realization, the sutures are not applied and the chewing function is re-established in a very short time. But, the greatest advantage of immediate loading implantology for patients is the possibility of having fixed teeth immediately. As for the cost, this does not vary as much compared to the deferred load technique. However, many dental clinics and immediate load implant specialists present a lower price in their advertising campaigns, yet they do not mention that the crown or prosthesis included in this price is provisional and that in fact the patient would have to pay an additional sum for the crown. definitive in 3-6 months.
In conclusion, deferred loading can always be done but not always immediate loading. To find out the opinion of our dentists for your particular case, request a personalized quote now.
