Dental clinic in Moldova. European hygiene and sterilization.

How much to invest in hygiene and sterilization is the choice of each dental clinic. Each clinic determines how much it is ready and can afford to invest in something that, at the first sight, does not bring money but waste it.

Often, because of lack of information, patients, make their choice based on prices, ignoring other aspects very important that stand behind the quality.

Although some of the sterilization rules vary from country to country, the general concept of sterilization and hygiene of a dental clinic has the same premises.

Regardless of the choice you make concerning the dental treatment and dental clinic in Moldova, make sure that all the rules are fully(not partially) taken into account during the treatment and after each patient.

Don’t be embarrassed about asking questions, your health and safety are more important than the 5 minutes you will spend to ask.

Disposable shoe covers

Somebody can consider it a trifle or even an element of marketing, but for us they are essential.

No matter how we wish to optimize our expenses, the shoe covers remain to be a must without exception.

Just imagine for a second that all patients, doctors, nurses and other auxiliary employees getting into the surgical cabinet wearing the outdoor shoes. Are you sure you are ready to be the next patient for the surgical intervention?

All surfaces of the cabinet and the dental chair have been treated with disposable sanitizing wipes.

HOW DO WE DO?

Disinfection should be done without exception

  • in the morning before the first patient,
  • in the evening after the last patient
  • and also after each patients treatment during the day.
  • We use 2 types of sanitizing wipes.

Disposable accessories and reusable pieces

The following accessories are changed before each patient:

  • a disposable glass with a mouthwash tablet which also contains antibacterial substances
  • the saliva ejector;
  • disposable trays for materials and tools.

WHAT DO WE DO MORE:

  • The turbine and the other pieces attached to the dental chair which are introduced in the patients mouth, in our clinic they are not simply cleaned with wipes but pass through all stages of instrument processing, sterilization and packaging.
  • All instruments are opened in front of the patient sealed in sterilization pouches.

Dental clinics fall into the category of institutions which are subject to many restrictions regarding the hygiene.

Even the floor has to meet some essential requirements: they should be safe when walking on it, easy to clean, disinfect and to treat with antibacterial substances.

WHAT DID WE DO MORE?

We chose to install the medical linoleum which has the following advantages :

  • This floor has no seals that hold dirt which could not be cleaned completely. We excluded the sandstone from the very beginning for the simple reason that it does not allow cleaning and disinfection between plates.
  • It is antibacterial and antifungal, meeting all medical standards.
  • It is resistant to different chemical agents.
  • It is a linoleum for intense use, it does not wear out and therefore maintains its antibacterial characteristics.

Disposable gloves

The disposable gloves are not sufficient for surgery interventions.

  • Be careful that the gloves are changed with every patient, not only disinfected.
  • If the nurse or the doctor stepped out of the cabinet or touched objects that cannot be processed or disinfected, they must change their gloves.

Important:

For surgical interventions the disposable gloves are not sufficient. In this case we provide disposable sterile gloves, packed separately in a single set.

Antibacterial lamps.

Opening the windows in the dental office can help with ventilation but this is not an antibacterial solution for the air.

Quartz lamps are a mandatory requirement for the medical institutions and must be switched on once in a few hours for some minutes, during which it is mandatory to get out of the dentist office.

WHAT DID WE DO MORE?

For this reason, to make sure the lamps are on in time, without having to stop the surgeries, we have chosen antibacterial lamps that can be set to work at certain times and can be switched on even when people are in the office.

Central ventilation.

While we're on the subject of ventilation, then you should know that this factor has its peculiarities.

We chose to do it in the right way Since the clinic’s planning, we have developed an integrated ventilation system that assumes that every few hours, the air in each room is completely changed. This type of ventilation has benefits for employees because it nourishes the brain with oxygen and prevents the circulation of microbes from one room to another, being expelled outside via the ventilation channel.

Even here we need to follow a protocol. All tools used during treatment/ intervention are stored in a transportation box, covered with a lid and immediately sterilized in order to avoid any possible contamination In this way we take care of our staff and no less important of our guests.

The stages of cleaning, sterilization, packaging and autoclaving of medical instruments.

When talking about this process, there is no place for laziness and ignorance. For our clinic and for our patients we choose SAFETY. That’s why we work following these steps :

  1. In the sterilization room people’s access is limited! We have 2 people who take care about these processes. We do not practice the method in which an assistant begins the sterilization process, then she leaves it because she has to assist the doctor, and another available assistant comes to finish the work begun by the first assistant. You do realize that in this way we cannot get the certainty that the work has been done according to disinfection and sterilization protocol.
  2. Except of other people who are directly involved in sterilization process, the head assistant is responsible for checking the instruments and the accuracy of the sterilization.. Therefore, we make sure that any mistake is ruled out of this context.
  3. All tools are carried in a box containing 6% hydrogen peroxide.
  4. Mechanical cleaning. Instruments are cleaned with a rough brush and detergent under running water to remove any residual material or blood. Brushes are changed daily to prevent bacteria overgrowth.
  5. Chemical cleaning. Further, we sanitize the medical instruments with concentrated solution for washing and disinfection for 30 minutes. Although according to the instructions the use of this disinfectant solution is sufficient and we can also omit some disinfection steps, we HAVE DECIDED TO DO SOMETHING MORE: We chose to follow all the stages, even if others would consider it a waste of time. In addition, to prevent microorganisms from getting used to a particular type of solution and becoming ineffective, we change the solution every six months.
  6. Immediately after, the instruments are placed in the 4D ultrasonic cleaner.. This means that instruments are well agitated under the waves from four directions. In this way dental materials or blood that infiltrates through the cracks of the instruments are detached and removed.
  7. If there are still some residues on tools, the instruments washing machine will surely clean everything. It has practically the same construction as a dishwasher, but it is more performing, and uses special washing and disinfection solutions.
    This acquisition was an extra expense for us, but we realized that it will offer additional safety for our patients.
  8. The washing cycle finished and the time has come for the inspection of each instrument.. All tools are placed on a sterile rag which is thermally treated and changed daily Each instrument is carefully checked from all sides and prepared for the next step.
  9. The test Azopiram. The last step we have chosen to make for extra safety is the Azopiram test or the authenticity test of the quality of cleaning and sanitizing, which confirms the lack of blood residue.
  10. Packaging of the dental instruments in sterile rolls.. The appropriate roll size is chosen and the instrument is placed in the package with great care, to avoid damage, then it is sealed thanks to the pre-heated sealer and, further, it is placed in the tray of the autoclave with the special test confirming that the cycle of the autoclave has proceeded according to the rules.
  11. Thermdisinfection – Autoclaving. When we chose autoclaves for the clinic, we opted for Class B autoclaves, which involves the sterilization of instruments at a certain temperature and a program providing a special processing for instruments as the previous person had been infected with hepatitis. Although for most instruments the heat treatment done in a S-class machine is enough, WE HAVE CHOSEN to avoid possible mistakes because of the human factor and all instruments go through the Class B autoclave,

Important:

All instruments were packed in sterile pouches, they are open in front of you, but was the package damaged? Unfortunately, these instruments can no longer be called sterile and should be repackaged and autoclaved again.

Important:

Also pay attention to the autoclaving date written on the package. Instruments are sterile only 20 days from the date of autoclaving.

Many of what we describe are in fact provided by law, but unfortunately many clinics, due to saving on additional staff in sterilization and control, dismiss some steps.

Everything costs a lot: special people in sterilization, better autoclaves, instruments washing machine, 4D ultrasonic cleaner, shoe covers, solutions, tests, time invested in the process, but your health costs even more.

We do not save on sterilization and hygiene.
