Zygomatic and classic implants. Patient rehabilitation with severe bone atrophy

The first patient with zygomatic implants in our clinic

Paolo was the first lucky patient who managed to have fixed teeth thanks to the zygomatic implants.

First things first…

Like most of our patients, Paolo arrived in Moldova with the purpose to save on complex dental treatments that unfortunately he couldn’t afford in his country.

He needed a complete rehabilitation and obviously he wanted fixed teeth. But after the first consultation with our specialists and the examination of the computerized tomography it was clear that he has zero chances of inserting classic implants on the upper jaw. The bone was really reduces and even 3-4 bone augmentations would have guaranteed the success of the implantation.

This is how the patient came at our dental clinic in Chisinau

The patient, was disappointed and decided to accept the less invasive option – Removable prosthesis as a definitive solution.

Whether it is luck, fate or coincidence, but exactly at that time our Italian partners proposed us to put in practice the zygomatic implants.

Paolo didn’t hesitate for a second and immediately decided to be the first patient in our clinic who would experience the benefits of the zygomatic implants.

From removable prosthesis to fixed teeth in just a few days

We managed to organize all the necessary for the intervention in a record time (less than 24 hours):

• Blood Tests, chest radiography and electrocardiogram of the patient;
• consultation with the anesthesiologist, that is mandatory for these kind of interventions;
• purchase of zygomatic implants and the necessary accessories.

The next day, the intervention was done by our oral and maxillofacial surgeon, in collaboration with the Italian colleagues.

Although in most cases the general anesthesia is at the patient’s choice, for a pain-free or comfortable treatment, in Paolo’s case this was really necessary.

Thanks to the general anesthesia, all implants were inserted in the both jaws in a single intervention.

Because the zygomatic implants are inserted into the zygomatic bone (which is just below the eye), the surgeon is not “allowed” to make a mistake. In general anesthesia the patient remains still during the intervention and the surgeons can work with maximum accuracy and attention.

The last remaining roots of the upper jaw were extracted, then 4 zygomatic implants and a classic implant were inserted in the frontal area to provide a better bridge stability during the implant integration period.

All the teeth from the lower jaw were extracted and 6 classic implants were inserted.

Provisional prosthesis and the final work

Paolo was pleasantly surprised when he discovered that apart from the fixed final work, he will have fixed provisional teeth within a few days after the surgery.
So, the patient could smile from the first days and feel the taste of the food of yesteryear. With these provisional teeth, the patient left Moldova (obviously after we removed the suture threads).

The provisional teeth were made from PMMA (milled acrylic CAD CAM). They are not very aesthetic, but absolutely functional and probably the most important, they are FIXED.

The patient returned to Chisinau after almost 8 months to complete the treatment.

All the implants on the upper jaw (including the zygomatic ones) were integrated and had a good stability. Instead, the last implant on the right side of the lower jaw wasn’t integrated. However, this didn’t stop us from making the final bridge on the other 5 stable implants.
Finally, the patient has 2 FIXED bridges made of metal-ceramic, each one composed by 12 teeth. It is a sufficient number of teeth to have a good chewing.
