How did Margaret get to Moldova?
Margaret cares a lot about her teeth, and she hoped to find a conservative doctor to save her teeth. In Romania, they proposed her a radical treatment with the extraction of all her teeth. Although the periodontitis has reached an advanced level, Margaret instantly refused the Romanian colleagues, because this was the only solution they proposed, with no other alternatives. Knowing that dental tourism is also practiced in Moldova, she decided to contact us for an additional opinion on her issue. Although she still was in that clinic in Romania, we started to organize her visit to Moldova. Thus, very soon, Margaret was already in Chisinau, in our clinic.
We managed to make Margaret happy and save a good part of her teeth. The remaining teeth with third degree mobility were replaced with dental implants. The teeth that remained and were saved were also mobile, but a little less. Under these conditions, it is difficult to guarantee that the teeth and the crowns will last a long time, but we didn’t rule out this possibility either. In the case of the patient, regular check-ups and more frequent hygiene can certainly prolong the life of the teeth and the entire work. This is not a problem for Margaret, because Belgium, her country of origin, is only 1 hour and a half away from Moldova (by plane).
Multidisciplinary approach
Being a complicated case, with several teeth in doubt, we convened a council for the least compromised solution. The multidisciplinary commission was attended by surgeons, prosthetists and periodontists. Together we decided to extract some teeth and replace them with implants and bridges in zirconium. Why zirconium? We chose this material, in order to prepare the teeth as less as possible. Thus the teeth remain alive and their life expectation increases. Zirconium compared to the metal has a white/ opaque color, respectively requires a thinner layer of ceramic to cover only its opacity and get a more natural effect.
Implant supported bridges were also made of zirconium, to keep the same shade of transparent white on both jaws on all teeth.
Also at our recommendation, Margaret chose a screw retained work on implants instead of the “cementation” method. MUA systems used for fabricating ”screw retained” constructions allow a perfect fusion of the prostheses without tension on the implants.
Modern technology – the right hand in limited time conditions.
Milling machine CAD / CAM was our indispensable friend and help in this case, because Margaret had very little time. With the help of this machine, we managed to accelerate the process of milling / making zirconium crowns. In just one week from the first impression, the patient was already at home smiling with her 24 beautiful, white and healthy teeth.
It was also possible to prepare the the crowns in so little time because because of the fixed temporary teeth that the patient wore for 6 months (the period of integration of the implants in the bone). Immediately after tooth extraction and implant insertion, we chose for immediate loading of implants due to their good stability in the bone. The gum was already shaped under the fixed provisional construction, so in the final stage we didn’t use the scalpel at all. The gum was in perfect health, well formed and ready for the impression.
Also due to the fixed temporary, the patient had managed to get used to the new bite (occlusion). So, when we made the first try in of the final work, it perfectly matched, without the need to correct or adjust it.
Although, time was pressing on us, you should know that we made sure that all the details were done perfectly, including the preparation of the teeth. Thanks to the microscope, the preparation process was fast, efficient, but most importantly – minimally invasive. The microscope allows an enlargement of the image by up to 24 units, which allowed us to remove as little healthy tissue of the tooth as possible, in a record time.
As well the lab technicians use the microscope in their work to create the most beautiful and refined teeth.
Margaret left our clinic with tears of happiness and promising us that she will return regularly for follow-up visits – extremely important, especially in her case for keeping her teeth as long as possible.
